Frequently, patients in environmental health out-patient units relate various complaints to their amalgam fillings. However, an association between the toxic exposure and the reported complaints appears plausible only in few cases. We investigated toxicological, allergological and psychological parameters in patients with amalgam-associated complaints and compared them to controls with similar numbers of amalgam fillings. Forty patients with health disturbances related to amalgam were compared to a control group without amalgam-associated complaints (n = 40), carefully matched for age, sex, and dental status. Mercury concentrations were analyzed in blood, saliva, and 24-h-urine. Atopic predisposition, determination of IgE, patch testing with amalgam and amalgam-associated metals and a psychometric assessment were performed in all participants. Mercury concentrations in blood or urine were similar in patients and controls. Atopic predisposition was markedly enhanced in patients (11/40) as compared to controls (5/40). Only one patient with a lichen ruber of the oral mucosa showed a contact sensitization to amalgam. Patients reported more psychic strain and higher depression scores than controls. Somatization disorders were found in 10 patients (25%) and in one control. Eighteen patients (45%) neither showed an atopic predisposition nor an influence of psychosocial factors. Toxic exposure to mercury does not appear to play a role in "amalgam disease". Since many of these patients are atopic without an "amalgam allergy", but with more psychic strain and notably more depression, the treatment should be focused on allergologic and psychological factors.

BACKGROUND: Possible adverse health effects due to mercury released by amalgam fillings have been discussed in several studies of patients who attribute various symptoms to the effects of amalgam fillings. No systematic relation of specific symptoms to increased mercury levels could be established in any of these studies. Thus, a psychosomatic aetiology of the complaints should be considered and psychological factors contributing to their aetiology should be identified. METHODS: A screening questionnaire was used to identify subjects who were convinced that their health had already been affected seriously by their amalgam fillings (N = 40). These amalgam sensitive subjects were compared to amalgam non-sensitive subjects (N = 43). All participants were subjected to dental, general health, toxicological and psychological examinations. RESULTS: The two groups did not differ with respect to the number of amalgam fillings, amalgam surfaces or mercury levels assessed in blood, urine or saliva. However, amalgam sensitive subjects had significantly higher symptom scores both in a screening instrument for medically unexplained somatic symptoms (SOMS) and in the SCL-90-R Somatization scale. Additionally, more subjects from this group (50% versus 4.7%) had severe somatization syndromes. With respect to psychological risk factors, amalgam sensitive subjects had a self-concept of being weak and unable to tolerate stress, more cognitions of environmental threat, and increased habitual anxiety. These psychological factors were significantly correlated with the number and intensity of the reported somatic symptoms. CONCLUSIONS: While our results do not support an organic explanation of the reported symptoms, they are well in accord with the notion of a psychological aetiology of the reported symptoms and complaints. The findings suggest that self-diagnosed 'amalgam illness' is a label for a general tendency toward somatization.

die zwei Studien kommen zu dem Ergebnis, daß kein Zusammenhang zwischen der Amalgambelastung und den Beschwerden bestehen. Sie gehören aber trotzdem in die Liste der Beweisstudien, da sie sehr schön die methodischen Fehler der Amalgambefürworter zeigen!

Man hat also die Quecksilberkonzentration in Blut, Speichel und 24-h-Urin zum Vergleich herangezogen. Chronisch kranke Amalgamis scheiden aber dort so gut wie nichts mehr aus. Das Quecksilber sitzt z.B. im Nervengewebe und kommt dort von allein nicht wieder heraus. Die einzige bekannte Substanz, die das Quecksilber dort wieder rausholen kann, ist die grüne Korianderpflanze. Auch vermisse ich eine Stuhluntersuchung und einen DPMS-Test.
Eine solche Herangehensweise ist unwissenschaftlich. Wenn ich das Für und Wider der Amalgamschädigungen untersuchen will, muß ich mich vorher mit den Argumenten der Amalgamgegner auseinandersetzen. Wer aber die Theorie nicht kennt, kann auch keine ordentliche Versuchsplanung durchführen.
Man hätte zusätzlich zu Blut, Speichel und 24-h-Urin den Stuhl, die Atemluft (unmittelbar nach Koriandergabe!) und den Schweiß untersuchen müssen, nach der Provokation mit DPMS und Koriander + Bärlauch/Knoblauch + Chlorella!
Im zweiten Text wird auch erwähnt, daß sich kein signifikanter Zusammenhang zwischen den Beschwerden und Anzahl und Oberfläche der Amalgamfüllungen ermitteln lies. Dies ist aber eine allgemein zu beobachtende Tatsache. Manche haben 10 Füllungen und keinerlei Probleme, andere haben zwei Füllungen und sind furchtbar krank. Die indiviuelle Empfindlichkeit und Entgiftungsfähigkeit ist hier entscheidend. Ich z.B. habe HPU und dadurch eine ordentliche Amalgamvergiftung, andere haben keine solchen Probleme mit der doppelten Anzahl Füllungen, weil deren natürliche Entgiftung tadellos funktioniert und sie auch keinen Zinkmangel haben. Dies hätte man aber vor der Studie herausfinden können!
Medikamente werden verboten, wenn heftige Nebenwirkungen bei einem Teil der Patienten auftreten. Dort wird nicht versucht, einen Zusammenhang zwischen der Dosis und den Beschwerden zu finden. Es reicht, wenn bei einem gewissen Prozentsatz der Patienten gefährliche Nebenwirkungen auftreten. Bei Amalgam ist dies aber merkwürdigerweise nicht der Fall...

hi biologe!

quecksilber lässst sich kaum im blut nachweisen, da es sehr schnell im körper an proteine in den körperzellen gebunden wird (depotbildung). nur bei akuten vergiftungen sind schwermetalle im blut nachweisbar.

ähnliches gilt für den urin: da schwermetalle nicht frei zirkulieren können die nieren diese auch nicht herausfiltern. anders verhält es sich, wenn chelatierende substanzen wie dmps oder korianderkraut verabreicht werden. nach der gabe sind bei belasteten personen recht bald im urin schwermetalle nachweisbar.

die quecksilberkonzentration im speichel steigt vor allem nach dem verzehr saurer speisen (z.b. zitrusfrüchte). die größte belastung für den körper geht allerdings ohnehin nicht vom speichel aus, da im darm nur geringe mengen quecksilber absorbiert werden, sondern das eigentliche problem sind die quesilberdämpfe, die bei jedem atemzug inhaliert werden. dieser effekt kann mit messgeräten, die quecksilberkonzentrationen in der luft feststellen können nachgewiesen werden.

allerdings kann es gut sein, dass menschen, die sich über amalgam in ihrem mund sorgen machen, tendenziell auch allgemein eher sorgenvoll sind. es ist ganz sicher ein optimales feld für hypochonder (da man ja quasi jedes symptom mit amalgam in verbindung bringen kann). andererseits kann sich eine amalgambelastung negativ auf die kommunikation der nervenzellen auswirken, somit z.b. den serotoninspiegel negativ beeinflussen und damit zu allgemeiner depressivität und hypochondrischen störungen etc. führen.

Gut, daß ich erst zu Mittag gegessen habe, bevor ich mich an diese beiden Texte gemacht habe :p ...

Ergänzung:

Studie 1 wurde von der Abt. für Medizinische Psychologie verfaßt, Studie 2 von der Abt. für klinische Psychologie...

Die Teilnehmer der 1. STudie wurden mit Pflastertest auf Amalgam bzw. amalgam-assoziierte Metalle untersucht. Es ist bekannt, daß der Pflastertest nicht nur bei Amalgam ziemlich unzuverlässig ist. Von einem LTT-Test ist keine Rede.

Immerhin hat sich hier ein Lichen-ruber-Patient unter 40 gefunden, bei dem auch eine Kontakt-Sensibilisierung auf Amalgam festgestellt wurde.

Folgerung der ersten Studie:
"Da viele dieser Patienten eine allergische Disposition haben ohne eine Amalgam-Allergie zu haben, dafür aber mehr psychischen Stress und vor allem mehr Depressionen haben, sollte die Behandlung dieser Patienten mit Schwerpunkt auf allergologische und psychologische Faktoren veranlaßt werden" (übersetzt).

STudie 2:

Durch einen Fragebogen wurden Patienten herausgefiltert, die davon überzeugt waren, daß ihre Probleme durch Amalgam entstanden waren. 40 dieser Patienten wurden mit 43 Patienten verglichen, die auf Amalgam nicht reagierten (wie hat man die wohl herausgefunden?). Alle Patienten wurden untersucht an Zähnen, allgemeiner Gesundheit, toxikologisch und psychologisch. (wie??)
Ergebnisse: die zwei Gruppen unterschieden sich nicht in Bezug auf die ANzahl der Amalgam-Füllungen, die Oberfläche der Füllungen oder Hg-Menge in Blut, Urin oder Speichel.

Konklusion:
Während unsere Ergebnisse keine organische Erklärung für die berichteten somatischen Probleme unterstützen, belegen sie sehr wohl eine psychologische Herkunft der berichteten Symptome und Beschwerden. Unsere Untersuchungsergebnisse legen nahe, daß eine selbst-diagnostizierte "Amalgam-Krankheit" eigentlich eine generelle Tendenz zur Somatisierung zeigt.

Also: was von vornherein angenommen wurde, wurde belegt, fragt sich eben nur, mit welchen Mitteln. Und auch mit welchen Patientenzahlen. Eine Teilnehmerzahl von 80 insgesamt scheint mir doch sehr wenig, zumal gar nicht klar ist, ob z.B. die Entgiftungskapazitäten von Leber und Niere mit untersucht wurden.

Und Betrüger setzen alles daran, diese Störung aufrecht zu erhalten und weiter zu verschlimmern. Es wird schamlos abkassiert. Das ist Fehlbehandlung und Körperverletzung.

Hypochondern könnte Psychotherapie helfen - wenn sie nur daran glauben würden.

Wer partout zum Hypochonder werden möchte, kann sich an großen Vorbildern orientieren: am Aufklärer Voltaire, an Charles Darwin, Thomas Mann und Rainer Maria Rilke.

Auch die Komiker Woody Allen und Harald Schmidt spielen sich mit hypochondrischen Bekenntnissen in die mitleidenden Herzen ihres Publikums.

Doch was sich so trefflich verulken lässt, ist für viele Deutsche eine alltägliche Qual: Bauchdrücken - ist das etwa Magenkrebs? Blaue Flecken - Leukämie? Stechen in der Brust - Herzinfarkt?

Neueste Untersuchungen offenbaren, wie sehr das Problem bislang unterschätzt wurde. Zwar gilt nach strengen medizinischen Kriterien nur als Hypochonder, wer seit mindestens einem halben Jahr grundlos davon überzeugt ist, schwer körperlich erkrankt zu sein, ohne sich durch den Arzt eines besseren belehren zu lassen.

Deutliche Krankheitsangst bei sieben Prozent

Unter diese Definition fällt nach Schätzungen weniger als ein Prozent der Menschen. Doch "Krankheitsängste" plagen weitaus mehr: Die Psychologen Gaby Bleichhardt und Wolfgang Hiller von der Universität Mainz haben fast 1600 Deutsche nach ihrer subjektiven Befindlichkeit befragt.

"Nach unseren Schätzungen leiden etwa sieben Prozent der Bevölkerung unter einer deutlichen Krankheitsangst", berichtet Bleichhardt. "Diese Menschen machen sich tagtäglich Sorgen um ihre Gesundheit und gehen sehr häufig zu Ärzten."

Ähnlich sieht es der Marburger Hypochondrie-Experte Winfried Rief. Er schätzt, dass zwischen zwanzig und fünfzig Prozent der Patienten beim Hausarzt unter somatoformen Störungen leiden, zu denen auch die Hypochondrie gehört.

Organisch gesund und trotzdem Rückenschmerzen

Als "somatoform" werden Beschwerden bezeichnet, für die sich keine "somatischen", also körperlichen Ursachen finden lassen. Die Menschen sind organisch gesund und klagen dennoch über Rückenprobleme, Gelenk- und Gliederschmerzen, Kopfweh oder Magen-Darm-Beschwerden.

Zu den Schmerzen kommt die ständig Sorge, eine unentdeckte, schwere Krankheit zu haben. "Viele Personen mit Hypochondrie oder unklaren körperlichen Beschwerden entwickeln im weiteren Verlauf das Vollbild einer Depression", sagt Rief.

Der Patient braucht dringend Hilfe. Das Gesundheitssystem aber ist auf diese Patientengruppe kaum eingestellt. "Die Medizinerausbildung und das ärztliche Verhalten sind daran orientiert, schwere Erkrankungen zu erkennen", erklärt Rief. "Der Arzt hat ja die Aufgabe, nichts zu übersehen und ist daher selbst sozusagen ‚hypochondrisch'"

Jahrelanger Ärztemarathon

Folgt auf Mammographie, Bluttest oder EKG dann die Diagnose "gesund", bringt das allenfalls kurzfristige Erleichterung. Dann keimt wieder der Zweifel.

Oft ist das der Beginn eines jahrelangen Ärztemarathons: Während es der Durchschnittsdeutsche laut Bundesgesundheitssurvey auf elf Arztbesuche pro Jahr bringt, suchen Patienten mit unklaren Körperbeschwerden den Arzt im Schnitt etwa dreimal so häufig auf, so eine Studie Riefs. Die Kosten dieses "Doktor-Hoppings" liegen daher auch drei- bis viermal so hoch wie beim Durchschnitt.

Die Forschung hat aufgeholt

Beim Vollbild einer Somatisierungsstörung ist gar mit den neunfachen Kosten zu rechnen. Zudem gehen somatoform Erkrankte im Schnitt sieben Jahre früher in den Ruhestand, sagt Rief.

Auch die klassische Medizin und Psychiatrie boten dem Leiden der "kranken Gesunden" keine wirkliche Linderung. Inzwischen hat die Forschung aufgeholt. Mit Psychotherapien, gegebenenfalls in Kombination mit medikamentöser Behandlung, lassen sich Krankheitsängste heute in den Griff bekommen (1).

Insbesondere eine so genannte kognitiven Verhaltenstherapie hat oft recht schnell Erfolg. Das konnten die Harvardmediziner Arthur Barsky und David Ahern jüngst in einer klinischen Studie zeigen (2).

Mehr als Hundert Patienten, die sich in Fragebögen als Hypochonder zu erkennen gegeben hatten, schickte man zum Verhaltenstherapeuten: In nur sechs ambulanten Sitzungen korrigierten die Teilnehmer nicht nur ihre Eigenwahrnehmung.

Sie lernten auch, wie sie ihr Verhalten aktiv verändern konnten. Zu den Strategien gehört etwa, nicht mehr ständig das Internet nach Krankheitsthemen zu durchforsten, Nachrufe zu lesen, immerzu den Blutdruck zu messen oder Arzttermine zu arrangieren.

Diese Therapie hatte dauerhaften Erfolg: Auch ein Jahr nach den Sitzungen zeigten fast sechzig Prozent der Teilnehmer eine deutliche Besserung. Die körperlichen Symptome waren zwar nicht verschwunden, doch hatten die Patienten damit zu leben gelernt.

Auch in Deutschland haben Ärzte gute Erfahrungen mit einem solchen Behandlungskonzept gemacht. So werden Menschen mit somatoformen Störungen - viele von ihnen mit dem Vollbild Hypochondrie - an der Medizinisch-Psycho somatischen Klinik Roseneck am Chiemsee erfolgreich mit kognitiver Verhaltenstherapie stationär behandelt (3).

"Die Behandlung passte nicht in ihr Glaubensschema"

Allerdings bleibt das eher die Ausnahme: "Bei der ambulanten wie der stationären Behandlung gibt es in Deutschland noch ziemliche Defizite", sagt der Marburger Fachmann Rief. Nachholbedarf sieht der Psychologe vor allem in der Ausbildung und Schulung von Therapeuten und Ärzten.

Aber auch wenn die Methode richtig angewandt wird, greift sie nicht bei allen Patienten. In Barskys Studie an der Harvard University versagte die Behandlung bei jedem vierten Studienteilnehmer.

Als sie erfuhren, dass ihre Krankheit psychische und keine organischen Ursachen hat, brachen sie die Therapie ab. "Die Behandlung passte nicht in ihr Glaubensschema", erläutert Barsky.

Hier liegt der entscheidende Punkt: "Die meisten Hypochonder würden niemals einen Psychiater aufsuchen", sagt der Forscher. Sie sind schließlich felsenfest davon überzeugt, dass ihre Krankheit "echt" ist.

Es gibt Hypochonder. Es gibt Ärzte, die alles auf die Psyche schieben. Es gibt Menschen, die schwermetallvergiftet sind und nach jahrelanger Suche nach den Ursachen ihrer Krankheit durch eine gezielte Schwermetallentgiftung wieder einigermaßen gesund geworden sind.
Hätten man bei diesen Menschen am Anfang ihrer Beschwerden als Arzt genau hingehört, einen DMPS-Test gemacht und vor allem ihnen von Anfang an gar nicht erst Amalgam- und andere Metalle in die Zähne eingebracht, wären sie erst gar nicht krank geworden oder schnell wieder gesund.

Fast alle hier haben u.a. auch eine oder mehrere Psychotherapien hinter sich, einschl. Klinikaufenthalten in Psychosomatischen Kliniken. Seltsam, daß Du von den Erfolgen der kognitiven Therapie schreibst, aber alle Amalgamis, die ich kenne (und das sind inzwischen viele) keinerlei Erfolg hatten mit einer Psychotherapie.

Es geht hier nicht um Hypochonder, es geht auch nicht um psychisch Kranke. Es geht um Schwermetallvergiftete, die höchstens durch ihre ständigen Beschwerden und das Nichtfinden einer Diagnose und einer Therapie auch noch psychische Probleme bekommen haben.

Ich habe gesehen, daß Du bei Wikipedia diese Liste auch gepostet hast. Hoffentlich gehen noch andere auf dieses Thema dort ein und werden nicht gelöscht...
