Das einzig Positive am episodischen Cluster-Kopfschmerz ist:
Irgendwann endet die Episode. Dabei ist es völlig egal, ob man irgendetwas dagegen unternimmt.
Placebowirkung mit einem Wirkungsgrad von bis zu 62% ist auch bei Cluster-Kopfschmerz nichts Ungewöhnliches, siehe z.B.:

Insbesondere Cluster-Kopfschmerz "Episodiker" gehen auch gerne einfach so in Remission, ohne dass dies irgendetwas mit einer Behandlung zu tun hätte. Das ist zum Glück der natürliche Verlauf der Erkrankung. Bei Cluster-Kopfschmerz "Chronikern" ist das nicht so?
Den besten Wirkungsgrad zur Behandlung ansonsten nicht behandelbarer chronischer Cluster-Kopfschmerzen hat es anscheinend, wenn man sich bei Professor Dr. Jean Schoenen in Liège auf die
Warteliste für eine Tiefenhirnstimulation setzen lässt:

While waiting for ethics committee approval, we assigned 12 candidate patients with an IHS diagnosis of refractory chronic cluster headache from the entire country
to a waiting list. After the first visit and changes in drug therapy made in some of them, nine patients went into remission and one was lost to follow-up.

Neun der zwölf ansonsten therapierefraktären chronischen Cluster-Kopfschmerzpatienten aus der Warteliste (75%!) gingen in Remission, bevor die Behandlung begann!!!
Kommentar in einer späteren Arbeit, da wundern sich auch die Experten:
Finally, as illustrated in a study of hypothalamic stimulation for intractable chronic cluster headache, putting patients on a waiting list for several weeks may suffice to induce remissions in some of them (26). This could be a placebo effect due to the new management by headache experts or to trivial changes in treatment regimens, or indeed the natural history of the condition.
