Step 1--Lower the Ammonia

Identify & address mitochondrial dysfunction
Optimize overall GI health:
Diet
Beneficial bacteria
Bad guy killers (caprylic acid, uva ursi, oregano oil, olive leaf extract, citrus seed extract)
And again:

Increase BH4 by lowering ammonia
Get minerals in balance - eat lots of fruit & vegetables;
potential minerals to consider:
Thorne Citramin II, Intramin Trace Minerals, MM lithium, iodine, zinc, magnesium, manganese, molybdenum
Make sure child can make BH4.
Potential supplements: minerals, NADH, ATP, TMG, Methylfolate (caution)

Step 3--Mop up the damage
Mitigate damage from superoxide and peroxynitrite (that body makes when BH4 too low--see chart)
Need antioxidants (A, C, D, E) and foods (double antioxidant fruits and vegetables)
Need polyphenols (black pepper, Pure Encapsulations Resveratrol, Liverlife). Use with No Fenol if child sensitive to phenols.
Low BH4 Fallout
Enzymes produced by the liver convert ammonia (toxic) into urea.
The cycling of Arginine through the various intermediates is what converts ammonia to urea. Urea is excreted by the kidneys.
